2. Consider whether you would like a very special date put on the adoption certificate (birthday, anniversary etc)
3. Click on the next page (Application Form) and complete the details as necessary.
4. If you would like to pay by credit card or debit card, complete & submit the form. If you are concerned about security, just submit the form without the card details and we will telephone you for them. Or you can of course use traditional mail. Just print off the form and send it in the post with your cheque.
