2. Consider whether you would like a very special date put on the adoption certificate (birthday, anniversary etc)
3. Click on the next page (Application Form) and complete the details as necessary.
4. If you would like to pay by PayPal, click on the button included on the form. This will take you to the PayPal site where you can safely initiate payment. If you wish to pay by credit or debit card, complete & submit the form. If however, you prefer to use traditional mail, please print off the form and send it in the post.
